Dermatology
We are out-of-network for all insurance plans and payment is due at the time of service. Insurance forms will be given for use to file a claim.
Regular Medicare is accepted as non-participating provider.
Rheumatology
BCBS insurance is accepted. However, all co-pays and deductibles will be collected at time of service. Laboratory and procedure charges may be separate and are subject to insurance plan co-pays and deductibles.

Medicare is accepted as a non-participating provider, which means the patient pays at the time of service and will be reimbursed directly by Medicare, depending on the allowable amount, deductibles and secondary insurance plans.
